induced moderate hypothermia for low cardiac output after pediatric cardiac surgery

چکیده
postoperative low cardiac output states are a major cause of postoperative mortality in infants and children following corrective cardiac surgery or congenital heart defects. whole body hypothermia has been used since 2001 in the management of these low output states when they are refractory to conventional modes of therapy. from december 2001 to april 2006, 25 cases were included in this study. the median (range) age of patients was 36.6 months (1 mo-19 y) with a median weight of 12.2 kg (3.5-44 kg). following cooling, there was a decrease in heart rate ( p p p p = 0.02). a decrease in platelet count was occurred ( p p = 0.18). fifteen of 25 patients survived to leave hospital. induced hypothermia does not appear to be associated with any complications and after the failure of all conventional treatments, it seems likely that the technique may have been beneficial to outcome in some patients.

The effects of magnesium supplementation on the incidence of cardiac complications after pediatric cardiac surgery
Background and Objective: The present study aimed to assess the role of magnesium sulfate (MgSO4) sup-plementation after cardiac surgery in pediatric patients, evaluating the incidence of cardiac complications, such as arrhythmia. Methods: In this study, 105 children scheduled for elective cardiac surgery were randomly divided into 3 groups. The first group, as the placebo group, received sali...

متن کاملHypothermia therapy after pediatric cardiac arrest.
BACKGROUND Hypothermia therapy improves mortality and functional outcome after cardiac arrest and birth asphyxia in adults and newborns. The effect of hypothermia therapy in infants and children with cardiac arrest is unknown. METHODS AND RESULTS A 2-year, retrospective, 5-center study was conducted, and 222 patients with cardiac arrest were identified. Seventy-nine (35.6%) of these patients ...
متن کاملManaging low cardiac output syndrome after congenital heart surgery.
Patients with congenital or acquired heart disease comprise a major diagnostic category for admissions in large pediatric intensive care units (ICUs) across the country, compromising 30% to 40% or more of admissions in many centers. Obviously, assessment and treatment of low cardiac output states among patients with heart disease can be life saving. Although some causes of low cardiac output af...
متن کاملHaemodynamic effects of levosimendan for low cardiac output after cardiac surgery: a case series.
INTRODUCTION Levosimendan is a new inotropic vasodilator for the treatment of decompensated heart failure. Compared to other inotropic agents, it has been shown to improve myocardial contractility without increasing oxygen requirements. However, experience with levosimendan in patients with low cardiac output after cardiopulmonary bypass is limited. In this case series we present the short-term...

A patient with preoperative pulmonary hypertension and a large left to right intracardiac shunt underwent surgical correction of a complex cardiac anomaly and required extracorporeal life support for unexplained ventricular dysfunction following the procedure. Following recovery, a fistulous connection between the right coronary artery and main pulmonary artery was demonstrated. Implications an...
